Brave Boxes from Nadeiah’s Warriors Foundation are care packages designed to bring comfort and joy to children battling cancer. To receive a Brave Box, families must follow these guidelines:
Eligibility Criteria:- The child must be diagnosed starting 2024 with cancer and currently undergoing treatment. - The child must be under 19 years old at the time of request. - The recipient must be located within the U.S. (or specify if international requests are accepted).
How to Apply: 1. Complete the Request Form - Fill out the official Brave Box request form below. - Provide the child’s name, age, diagnosis, date of diagnosis, and treatment status. - Include a parent/guardian’s contact information, please, include shipping address.
2. Medical Verification - A doctor’s note or hospital verification may be required to confirm the child’s diagnosis and treatment.
3. Personalization Details - note preferences such as favorite colors, characters, or activities to customize the Brave Box.
4. Submission & Processing - After submission, the request will be reviewed. - If approved, families will receive a confirmation email with an estimated delivery time.
Important Notes: - One Brave Box per child (unless otherwise stated). - Boxes are free of charge, funded by donations and sponsorships. - Delivery times vary.
